Vancomycin-Resistant Enterococci and Carbapenem-Resistant Enterobacterales in Epidemiological Surveillance Cultures

Abstract

The high number of multidrug-resistant bacteria (MDR), such as carbapenem-resistant Enterobacterales (CRE) and vancomycin-resistant Enterococcus (VRE), associated with the scarcity of effective therapy, represents one of the greatest challenges for the control of Care-Related Infections the health. The epidemiological surveillance cultures have an important role in the prevention and control of these infections. This study aimed to evaluate the prevalence of VRE and CRE, in epidemiological surveillance cultures, of hospitalized patients, as well as their distribution by gender and hospital sectors. A retrospective descriptive cross-sectional study was carried out in of a teaching hospital in the city of Juiz de Fora, MG,  from June 2021 to July 2022. 1090 rectal swabs were cultured, of which 277 (25.4%) were positive for MDR bacteria, 6.9% positive for VRE, 8 .9% positive for CRE and 9.6% positive for non-fermenting gram negative bacilli MDR. The VRE and CRE phenotypes were more isolated in male patients with 60.5% and 61.9% respectively. The wards had 15.7% positivity for VRE and CRE, with 7.2% for VRE and 8.5% for CRE. In the intensive care unit (ICU), 18.1% of the swabs were positive, with 4.3% for VRE and 13.8% for CRE.  The prevalence of CRE and VRE was 8.9% and 6.9%, respectively. Both phenotypes were more isolated in male patients and in those who were hospitalized in ICU.
