Linfócitos infiltrantes de tumor no microambiente tumoral: relevância diagnóstica, prognóstica e terapêutica.

Resumo

O microambiente tumoral (TME) constitui um ecossistema complexo, formado não apenas por células neoplásicas, mas também é composto pela matriz extracelular, vasos sanguíneos, células do sistema imunológico e múltiplos fatores solúveis que interagem de maneira dinâmica. Entre esses componentes, os linfócitos infiltrantes de tumor (TILs) destacam-se como mediadores centrais da resposta imune antitumoral, cuja presença, densidade, distribuição e fenótipo têm sido associados a melhores desfechos clínicos em diversas neoplasias. Apesar disso, sua avaliação histopatológica ainda carece de padronização diagnóstica. Esta revisão analisou estudos publicados de 2018 a 2025, enfatizando e os mecanismos de evasão imunológica das células tumorais e o papel da resposta inflamatória na progressão do câncer. As evidências sugerem que a caracterização detalhada dos TILs pode auxiliar na estratificação prognóstica, seleção de possíveis candidatos à imunoterapia e no aprofundamento do entendimento das interações imune-tumorais no TME. A incorporação dessa análise à prática anatomopatológica representa um avanço relevante na oncologia, com potencial para impulsionar estratégias terapêuticas mais personalizadas e eficazes.
